First Visit Patient Information
We are very excited that you are considering joining our practice!
Our philosophy is that an educated patient is a better patient. At your initial examination we will go through your mouth and list any and all points to improve, both for your oral health and any points you would like to change about your smile or teeth. We prioritize the list to develop a treatment plan that is best for you.
One of our staff will then research your financing options and provide you with a disc to take home explaining the details of the procedures recommended in your treatment plan.
We want you to look at your dental treatment as an opportunity to get the natural, healthy smile you want. Our job is to help you get it.

Patient History Form
Please download both of our new patient forms*, Patient Registration and Medical History. You can fill them out and bring them to your exam or email them to greatsmiles@davidcrumptondds.com. This will give Dr. Crumpton and his staff a good start on the information that they need to put into your file.
